Role Overview

As a Wound Care Physician Assistant in a Skilled Nursing Facility , you’ll provide high-quality, patient-centered care by establishing a plan of care, change dressings and perform medically necessary debridement. You’ll collaborate with interdisciplinary teams to develop and implement personalized care plans, monitor patient progress, and respond to acute changes in condition.

Key Responsibilities

• Visit patients throughout the Brooklyn area, all residing in Skilled Nursing Facilities
• Assessment & Diagnosis: Conduct comprehensive evaluations of wounds, including staging, measuring depth and severity, and identifying complications like infections or necrosis2.

• Treatment Planning: Develop individualized wound care plans, which may include prescribing medications, ordering diagnostic tests, and selecting appropriate dressings or therapies.

• Procedural Care: Perform bedside treatments such as conservative sharp debridement (where permitted), and monitor wound healing progress.

• Prescriptive Authority: When medically necessary, prescribe antibiotics, pain management medications, and advanced wound care products.

Qualifications

• Current Physician Assistant license in New York
• Active DEA License
• Wound Care Certified (WCC)
• 1+ years of clinical experience in wound care
• Familiarity with electronic documentation platforms
• Compassionate, patient-focused approach
