The SCC ISNP program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The Optum SCC APC provides care to our highest-risk health plan members and is part of an interdisciplinary team. Optum is transforming care delivery with innovative and personalized care. Primary Responsibilities:
- Conduct comprehensive assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation of acute and chronic medical and behavioral conditions
- Communicate and collaborate with the interdisciplinary care team
- Conduct quality advanced illness and advanced care planning conversations
- Provide members and caregivers with counseling and education
- Utilize advanced clinical nursing expertise, knowledge of geriatric/chronic disease management, and the long-term care industry to provide best in class care to members
- Foster and develop a culture of clinical expertise
- Anticipate customer needs and proactively develop solutions to meet them

Required Qualifications:

Certified Nurse Practitioner through a national board
- For NPs: Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult/Family or Gerontology Nurse Practitioners (ACNP), with preferred certification as ANP, FNP, or GNP
- For PAs: Graduate of an accredited Physician Assistant degree program and currently board certified by the National Commission on Certification of Physician Assistants (NCCPA)
- Active and unrestricted license in the state which you reside
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
- Ability to gain a collaborative practice agreement, if applicable in your state
- Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions
- Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours (after 5 pm, on weekends, and/or holidays) not to exceed 960 hours in a calendar year

Driver's license and access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Preferred Qualifications:
- 1+ years of hands-on post grad experience within Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records
- Ability to develop and maintain positive customer relationships
- Adaptability to change
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $43.22 to $77.21 per hour based on full-time employment. We comply with all minimum wage laws as applicable
