Position Description


Job Title: Advanced Practice Clinician - Nurse Practitioner / Physician Assistant
Location: Hartford, CT (Travel between multiple facilities in the Hartford area).
Schedule: 
  • Monday - Friday
  • 8:00 AM - 5:00 PM
Duration: Approx. 6 months (FMLA coverage)
Hours per Week: 40
Dress Code: Medical attire

Position Overview:
Seeking a dedicated and compassionate Advanced Practice Clinician (APC)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to join the Senior Community Care (SCC) program. This integrated care delivery model focuses on providing longitudinal, facility-based care to high-risk health plan members. The APC will work as part of an interdisciplinary team to manage complex medical and behavioral conditions, support advanced care planning, and improve patient outcomes.
This role offers flexible scheduling with no on-call, weekend, or holiday requirements, promoting a healthy work-life balance. 
Primary Responsibilities:
  • Conduct comprehensive patient assessments in facility settings.
  • Manage acute and chronic medical and behavioral health conditions.
  • Ensure accurate documentation and coding (ICD-10).
  • Collaborate with interdisciplinary care teams to coordinate treatment plans.
  • Lead advanced illness and care planning discussions with patients and families.
  • Provide education and counseling to patients, caregivers, and facility staff.
Required Qualifications:
  • Active and unrestricted NP or PA license in the state of residence (or ability to obtain by start date).
  • NP: Graduate of an accredited MSN or DNP program; board certified by AANP or ANCC in one of the following:
  • Family Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontology Nurse Practitioner
  • Adult-Gerontology Acute Care Nurse Practitioner
  • PA: Graduate of an accredited PA program; board certified by NCCPA.
  • Current DEA license and prescriptive authority.
  • Ability to meet physical requirements (lifting 30 lbs, navigating stairs, kneeling for assessments).
  • Reliable transportation for travel between facilities.
  • Ability to obtain a collaborative practice agreement (if applicable in your state).
  • Full COVID-19 vaccination required (per federal, state, and client regulations).
Preferred Qualifications:
  • 1+ year of clinical experience (long-term care setting preferred).
  • Experience managing patients with complex behavioral, social, or functional needs.
  • Knowledge of geriatrics, chronic illness, and end-of-life care.
  • Proficiency in electronic medical records and documentation systems.
