Job Title: Advanced Practice Clinician - Nurse Practitioner / Physician Assistant
Location: Bronx, NY
Shift: 8:00 AM - 5:00 PM
Work Days: Monday to Friday
Travel Required: Yes - 100%
Travel Area: Bronx, NY
Facilities Visited: 3-4 per shift
Multiple Locations or Home Visits: Yes - Senior Living Facilities

Job Summary:
Seeking an experienced Advanced Practice Clinician (APC) - Nurse Practitioner or Physician Assistant - to join a longitudinal, integrated care delivery team serving high-risk health plan members in facility settings. This role offers flexible scheduling with no on-call, weekend, or holiday requirements, supporting work-life balance while delivering innovative, personalized care.

Primary Responsibilities:
  • Conduct comprehensive patient assessments
  • Manage acute and chronic medical and behavioral conditions
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Lead advanced illness and care planning conversations
  • Provide education and counseling to patients and caregivers
Required Qualifications:
  • Active and unrestricted license in your state of residence (or ability to obtain by start date)
Education:
  • NP: Master of Science in Nursing or Doctor of Nursing Practice
  • PA: Accredited Physician Assistant degree
Certification:
NP: Board certified by AANP or ANCC in one of the following:
Family Nurse Practitioner
Adult Nurse Practitioner
Gerontology Nurse Practitioner
Adult-Gerontology Acute Care Nurse Practitioner
PA: Board certified by NCCPA
Current DEA licensure/prescriptive authority
  • Physical ability to lift 30 lbs, navigate stairs, and perform assessments
  • Reliable transportation
  • Ability to obtain collaborative practice agreement (if applicable)
  • Full COVID-19 vaccination (required by UnitedHealth Group policy)
  • Must pass a drug test prior to employment
Preferred Qualifications:
  • 1+ year of clinical experience (long-term care preferred)
  • Experience with complex behavioral, social, and functional patient needs
  • Knowledge of geriatrics, chronic illness, and end-of-life care
  • Proficient in electronic medical records and documentation
