Nurse Practitioner/ Physician Assistant – House Call Primary Care (Pensacola or Milton)

Full-Time | $100,000 – $130,000 annually | Mission-Driven Team

We care primarily for chronically ill, homebound older adults who thrive when they receive attentive, personalized medical care where they feel safest. If you feel called to serve this population with excellence, we would love to meet you.

What You’ll Do

- Provide comprehensive primary care to adult and geriatric patients in their homes.

- Average 8 visits per day (or as many as you can comfortably and safely care for).

- Manage chronic conditions, perform routine assessments, and coordinate patient care.

- Collaborate with our supportive clinical and administrative team.

- Document visits electronically and maintain high clinical standards.

What We’re Looking For

- Active Florida APRN license

- Experience in Adult Primary Care (geriatric experience a plus)

- A heart for serving chronically ill seniors

- Strong clinical judgment and independent practice skills

- Comfort with travel between patient homes in Pensacola or Milton

- Reliable documentation and communication skills
