Position Description
Position Summary:
The Colorado Rural Opioid & addictiOn Treatment System (ROOTS) is seeking a compassionate, experienced, and dedicated Advance Practice Provider (APP) to join our dynamic multidisciplinary team, in providing rural addiction care. This position will play a key part in delivering holistic and integrated inpatient and outpatient care for individuals with substance use disorders, and (to a lesser extent) chronic pain in rural communities. The APP will provide assessment, diagnosis, treatment, and ongoing care for patients engaged in our new TeleROOTS addiction practice, including providing medications for addiction treatment (MAT), treatment for infectious diseases (HIV, Hep C), treatment of pain, inpatient / withdrawal management consults and integrated behavioral healthcare. While addiction medicine or pain management experience is preferred, other qualified APPs will be considered.
Key Responsibilities:
- Perform telehealth or video visits for outpatient and inpatients.
- Provide in-person patient care during up to four clinical site visits each month at multiple hospitals and clinics on the Eastern Plains.
- Perform virtual consultations or rounding for patients admitted to the hospital or withdrawal management units.
- Conduct comprehensive patient assessments including medical intakes, medical history, substance use history, and risk assessments.
- Prescribe and manage medications for addiction treatment, including buprenorphine (Suboxone, Brixadi, Subclade), naltrexone (Vivitrol), methadone (if applicable), and other relevant medications.
- Prescribe medications for pain management in partnership with medical director including multimodal agents, buprenorphine products (Butrans, Belbucca, SL buprenorphine), full agonist opioids.
- Perform office-based pain management procedures such as trigger point injections and joint injections.
- Monitor treatment progress, adjust care plans as needed, and provide relapse prevention support.
- Collaborate closely with addiction counselors, case managers, social workers, and other healthcare providers.
- Provide education to patients and families regarding addiction, recovery, medications, and harm reduction strategies.
- Maintain accurate and timely documentation in electronic health records (EHR).
- Participate in quality improvement initiatives, staff meetings, and continuing education.
- Support local providers with consultation and technical assistance.
Qualifications:
- Required:
- Current and unrestricted Nurse Practitioner or Physician Assistant license in Colorado.
- Active and unrestricted DEA license
- Minimum of 2 years of clinical experience in addiction medicine, pain medicine, general medicine, hospital medicine, emergency medicine, or family medicine.
- No significant medical board actions or suspensions against medical license.
- Preferred:
- Experience with medications for addiction treatment (MAT).
- Experience with pain management.
- Experience in behavioral healthcare or knowledge of co-occurring mental health conditions.
- Experience working in integrated or team-based care models in rural areas.
Compensation & Benefits:
- Salary range $110,000 - $130,000
