This is a full-time on-site role for a Mobile Wound Care Nurse Practitioner based in the Los Angeles Metropolitan Area. The Nurse Practitioner will be responsible for providing specialized wound care services, including assessment, diagnosis, treatment, and management of chronic and acute wounds. Additional responsibilities include ostomy care, patient education, treatment planning, and collaboration with other healthcare professionals to provide comprehensive care. You will travel between patient locations to deliver care directly at their residence or specified care facilities.

• Expertise in Wound Care, including the ability to assess, treat, and manage acute and chronic wounds
• Knowledge and experience in Ostomy Care processes and treatment, including managing patients with ostomies
• Advanced practice in Nursing and Medicine, with a focus on patient-centered care and evidence-based practices
• Active Nurse Practitioner license, with certifications in Wound, Ostomy, and Continence (WOCN) care preferred
• Strong communication, interpersonal, and organizational skills
• Experience providing care in mobile or in-home settings is an advantage
• Ability to work independently and travel within the Los Angeles Metropolitan Area
