Associate Value-Based Care Outcomes Specialist (AVOS) - Houston, TX

Job Summary

The Associate VBC Outcomes Specialist is not a "fill-in" clinician; you are the Special Forces Vanguard treating in high-priority zones. Your mission is to establish a strong trust in territories where patient volume is building. You are an elite, W2-employed Nurse Practitioner or Physician Assistant who executes the Company's proprietary Value-Based Care (VBC) protocol with absolute precision.

This role is designed for the "Hunter/Healer"—a clinician who wants to treat patients, prove the model, and earn their seat as a Senior VOS. You are not paid for hours; you are paid for outcomes. Your objective is clear: Manage your initial cohort of patients to the "VBC Outcome" (healed wound, no readmission) and rapidly build your census to trigger your automatic promotion to the Senior Salaried Team.

Responsibilities

1. Strategic Outcome Execution & Protocol Dominance (The Hunter/Healer Mandate)

  • Precision Mission Ownership: Assume absolute clinical ownership for all assigned patients in the "Growth Zone." You own the episode from admission to successful "VBC Outcome" (healed wound, zero readmission).
  • Uncompromising Protocol Adherence: Execute the Company's proprietary, standardized VBC clinical pathway with 100% fidelity, including comprehensive assessment, precise diagnosis, and execution of advanced procedures (e.g., sharp debridement) strictly according to the single-source formulary.
  • The "One-Visit" Mandate: You are required to maintain clinical efficiency by adhering to the standard frequency of one in-person visit per week. Any additional visits must have explicit clinical justification and prior authorization from the Lead VOS. Unauthorized visits are not compensable.
  • The Anti-FFS Principle: Operate strictly within the protocol-driven W2/Piece Rate framework. The focus is the VBC Outcome, not the accumulation of billable procedures.

2. Data Discipline & Formulary Stewardship

  • Real-Time Documentation & Proof of Value: Complete all clinical documentation within the EHR immediately following the encounter (no later than end-of-day). Timely documentation is the "Proof of Life" for the billing cycle and is critical for VBC Outcome tracking.
  • Inventory & Reconciliation Command: Adhere with uncompromising rigor to the strict, single-source supply and CTP (Graft) formulary. Meticulously track and document every graft and supply used, ensuring complete transparency and reconciliation. You are accountable for the Variable Cost of Goods Sold (COGS) on your specific patients.
  • Asset Deployment Integrity: Perform all procedures and apply all clinical assets only in the exact manner, frequency, and quantity explicitly specified within the VBC clinical pathway.

3. "Path to Senior" Growth & Definitive Authority

  • Outcome Focus & Performance: Your performance is measured solely by the VBC Outcome Success Rate (Healed + No Readmission). You are paid to heal, not just to visit.
  • Census Building: Work in tandem with the VBC Account Executive (AE) to rapidly onboard new patients, leveraging your clinical excellence and "Radical Transparency" as the primary drivers of referral growth.
  • Protocol Champion: Act as the vanguard for the VBC model, consistently championing the commitment to data-driven, high-quality care to establish the VOS and the Company as the definitive, most trusted source of truth.
  • Strategic Collaboration: Engage with the Director of Clinical Outcomes and Regional Lead VOS for all caseload management, complex patient escalations, and performance reviews.

4. Ethical & Professional Compliance Command

  • Unyielding Compliance: Adhere strictly and without exception to all company policies, procedures, the Code of Conduct, all clinical compliance frameworks, and HIPAA regulations.
  • Ethical Veto Power: While protocol-driven, you retain the professional and ethical obligation to immediately exercise sound clinical judgment and escalate any patient safety concerns to the Director of Clinical Outcomes.
  • Brand Representation: Represent the company in all external and internal interactions with the utmost professionalism, integrity, and positive conviction.

Key Performance Indicators (KPIs):

  • Primary: VBC Outcome Success Rate (Healed + No Readmission).
  • Secondary: Protocol Adherence Rate (100% Mandatory).
  • Growth: Time-to-25 Census (Speed to Promotion).

Compensation Structure: The "Vanguard" Plan

This is a performance-based W2 role. You are an employee, not a freelancer, and you are paid for specific, authorized production.

1. The Visit Rate (Piece Rate)

  • Rate: $100.00 per Completed In-Person Patient Visit.
  • Definition: Paid for face-to-face clinical encounters where a billable note is generated.
  • Guardrail 1 (No-Show/Drive Time): Compensation is strictly for the clinical encounter. The Company does not pay for drive time, mileage, administrative time, or visits where the patient is not home/refuses care ("No-Show").
  • Guardrail 2 (Utilization Control): The authorized protocol is 1 Visit Per Week. Any visits beyond this frequency must be pre-authorized by the Lead VOS. Unauthorized visits are not compensable.

2. The "VBC Healing Bonus" (The Upside)

  • Bonus: $615.00 per Successful Episode Closure.
  • Metric: Paid when a patient episode under your management results in a confirmed "VBC Outcome" (Healed + Zero Readmissions).
  • Eligibility: The patient must be on service for a minimum of 30 days. Readmissions within 30 days of discharge trigger a clawback.
  • Why this matters: You are paid to heal, not just to visit. A 5-patient caseload that heals successfully generates $3,075 in bonuses on top of visit pay.

3. The "Path to Senior" Promotion Trigger

  • The Hook: This role is a bridge to the Senior VOS career track.
  • The Trigger: Upon reaching and maintaining an active, compliant census of 25 Engine 1 Patients for 30 consecutive days, the Associate Clinician will automatically convert to the Senior VOS Compensation Package:
  • --- Base Salary: $135,000 / Year
  • --- Benefits: Full Medical/Dental/Vision + 401(k).
  • --- PTO: Unlimited Paid Time Off.

Qualifications

  • Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in state of practice.
  • "Wartime" mindset—you do not need hand-holding; you need a target.
  • Preferred prior experience in home health, wound care, or hospice

  • Strict Protocol Adherence: "The Associate VOS acknowledges that despite the per-visit pay structure, they are a W2 employee required to follow strict clinical protocols. Deviating from the formulary or visit frequency without authorization is grounds for immediate termination."
  • Conversion Clause: "The Company commits to converting the Employee to full-time Senior status ($135k Base) effective the first pay period following the confirmation of the '25 Patient / 30 Day' census metric. This conversion is guaranteed based on performance.
